Medical translation Czech to Polish or English

Hi, I have some documents from the hospital that I would like to translate. There are 3 pages of text, it doesn't need to be certified, just for personal use.

I would like to have it ready before 15th of October. I prefer translation from Czech to Polish, but I will be also happy with Czech to English translation. Thanks!
