Performance Review record-keeper

Hello - I am going into a 2-hour Performance Review (at my request) with my General Manager, who agreed to a formal review (although he is very pressed for time). I have a vision impairment which can slow down accurate record-keeping, especially when the light conditions are not ideal.
The reason I requested the P.R is because there have been several confusing incidents & I am not clear on what exactly are the problems.
I need someone to take an accurate record in MS Word and then email to me the completed conversation record at session end. Will pay for the 2 hour session plua an agreed travel expense.
