PHP web form project to collect shipment detail from customer (repost)

Hi Freelancer ,
I want to create web form for my customer to filling their food shipment before enter to the US . This’s my requirement
1. Simple , Clean and Clear form . Support mobile view ( responsive ) .
2. In total will has 5 Main Step .
2.1 Filling about transportation type
2.2 Filling about shipper / sender
2.3 Filling about Receiver / Importer
2.4 Filling product detail in shipment
2.5 Validate customer coupon code
3. Step 1-4 can save setting as profile and allow to set multiple profile
4. Before start to filling form , customer must to login with social account button ( Facebook , Gmail or create new account with easy form only email and password need ) . Then we can store all profile for each customer
5. Freelancer must to design database schema . Mysql is prefer .
6. For the first phase , I want to do only first step “ Filling about transportation type “ . If everything go well , I will hire again for rest of step.
7. In my flowchart at step 1.b.2 , 1.b.3 and 1.d . You must store US state code , Port code and IATA code in database table .
8 After first phase is finished . Please send me php script and .sql file .
Please check my flow chart carefully before you bidding . You must to have a lot of knowledge to complete my project for example you must to know how to validate airway bill number .
