Now we’re looking for a Care Coordinator who actually wants to make an impact.

Your Role

You’ll be the first point of contact for new participants and help guide them into meaningful work.

• Run Zoom onboarding calls with new participants
• Understand their goals (especially employment)
• Place them into the right Leafe’s program
• Create service agreements
• Work closely with families and support coordinators
• Help participants move toward real employment outcomes

What We’re Looking For

• Experience in NDIS or disability support
• Strong communication skills (especially over Zoom)
• Ability to guide conversations and build trust
• Passion for employment outcomes (not just “hours”)
• Organised and able to manage multiple participants
