Zoho Creator, add button to report which sends information to zoho crm

I have created a report using zoho creator which is embedded into my website.

What I am hoping to create is a button "enquire here" which sends the information from the report into a pop up(webform) and then gathers there email and first name, along with the information from the report and sends to the crm as a lead.
You should be proficient at deluge and zoho creator.
