Need a webservice XLST, XSD programmer

I have XSD definition.
I have a webservice definition.
I have a form in CSS and a script that is calling webservice.
Form is working fine.
Webservice is working fine in a form based in CSS.
Then form was moved to a bootstrap enviroment, same form, same fields.
Need to copy-setup the webservice code in new form developed in bootstrap.
Webservice returns fields. That fields need to be saved in MySQL
Final step is genera a PDF with information saved in table.
