A Short Web/Mobile Form for Clients to Complete

I am looking for a freelancer to create a short web/mobile form for clients to complete. The purpose of the form is to gather client information for their mortgage application.
The form should include a file upload option, allowing clients to attach relevant documents. Documents provided should be sent to us in some secure manner that we can discuss.
The form will also need to collect Credit Card Information from the client along with a signed authorization for a charge for their credit report.
Once the application is complete, it should send an e-mail notification to our team along with the data collected in XML format. We can provide a sample XML that would make for a template of where the information collected should appear on the XML file.
I would like the form to have a professional and formal appearance, reflecting the seriousness of the information being collected.
We would like to be able to provide the files that make up this form to our web developer for integration with our website that is currently in development.
