EPI image name conversion from XML field

We would like a program with an interface that browses to a 2 file locations - 1. is to an image file folder. - 2. is to an XML file folder. The image name will take the starting image and add an underscore and last name from the corresponding XML file. The new file name would be saved in a newly created directory in the image file folder.
