It's up to you how you go about it, whether you want to make a new app/plugin or build on top of the current one.
I'm not sure what the best way to implement #3, but if we can pass that into meta fields inside of shopify that would be awesome. If it's not possible, add it to the order notes. ShopWP has a function for the order notes.
