custom order plugin

I’m looking for a custom order form that integrates with WooCommerce. This form will have 3 fields where a customer can input data: Product Name, Quantity, and Price. There will also be a Add/Delete button to add or remove products as needed, and a Add to Cart button. After a customer fills out the form with their products, clicking on the Add to Cart button will add each product as a special product with their name, quantity, and price to the cart/checkout.
