2 x Wordpress site fixes -- 2

Hi Everyone,
I have two issues I need fixed on one of my pages on my wordpress site
The page of concern is: https://www.bradnewtonfitness.com/macronutrient-calculator/
(1) The calculator on the page is no longer working (all I did was "duplicate" the page and now it is not working)
(2) To remove the white space (see attachment) on the same page
