Wordpress developer for creating two unsubscribe pages for our website

We are commencing an email marketing campaign for promoting the Call For Entries for the 13th LMAAGS with our email database using an email marketing service. As part of the email promotions, we would be having the following message at the bottom:
“Note: If you no longer wish to receive emails regarding the 13th LMAAGS, please click this link (LINK #1 TO WEBPAGE #1) to opt-out. If you would like to unsubscribe from all future emails, please click this link (LINK #2 TO WEBPAGE #2)“
For this, we require two basic web-pages to be built. Here are requirements for this:
- Both pages should be identical.
- Important: User should have to enter their email ID to unsubscribe. This should be stored at a database in the backend
- User needs to select one option from list of options (see reference attached)
After the user clicks unsubscribe, we need a success page like the attached reference.
We are planning to send 4 emails as part of this campaign, 3-4 days apart, one after another.. We will require an export of both these databases before each email is sent. The format of this database needs to be (example contains dummy data) (see attached reference):
We will then manually clean our database before sending the next batch of emails.
