Wordpress Sign-In Form with Signatures

Hi, I am interested in developing a website that allows users to create "Sign-in forms". Each sign in form should be customized for the user (Unique header, space for logo, name of form, date field, etc.) and have a unique link. The sign in form must have 3 fields for each signer: Date, name, and e-signature. Each form should be set with a certain number of possible signers. For example, one form may be set up with 10 possible signers, while another might set it for 25 possible signers, etc. The creator of the form should be able to access the form later and download the form with all data including e-signatures as a pdf.

I can provide examples of what I would want the export to look like and can provide more specifications. The wordpress website is already set up, no need for any designs, just need someone to develop the above listed functionality.
