1. Customize Gratity Forms WordPress Plugin
2. Fields to add

EMP ID (Autocomplete)
EMP Name
Manager Name (Autocomplete)
Star Rating 5/5
Project Details
Project Name

3. Enter the EMP ID in the EMP ID Field and selected ID represented EMP Name & Manager to to added
4. If we Enter the Manager Name, EMP Name & ID to be displayed in the Field
5. Star Rating to be added
6. Once all the fields are set and submit the form.
7. After submittion Email Notification to be sent
