Wordpress Contact Form Signature from word doc
Budget: $10 – $30 USD
This is simple project for someone who can create wordpress contact forms with signature from word doc file.
I just need to take a word document and convert it into an online form with signature.
Currently the website already has installed formidable plugin but this plugin DOES NOT have signature option.
You can use whatever drag and drop plugin like formidable, gravity forms, etc for this project.
If i have to i will buy a premium plugin like this one.

I would prefer you to have this option and install your plugin with signature and add your license as it makes it easier.

So i need the form very uniform in css and layout. quick simple to fill out. Once they will it out they click "signature" and date.
It is really only 1 form and the rest is mostly sign and date and name and email.
Data will be saved dynamically into backend of wordpress. Website administator will get get confirmation email with link to login to view the submissions so the content is not sent over email.
Can you do this job quickly and in 1 day?
