Wordpress: Add and style a note to display on all product pages

Hi there. We would like to add a header and a note on all product detail pages. Or alternatively a button on the product detail page which opens up a small enquiry form. This will consist of several fields.
Subject to cost we would also like to add a ups bar to our website above the main slider banner. Response dive view ie on mobile should be considered no it should display one usp at a time like a small slider.
