WordPress Responsive Multi Step Survey Form jQuery AJAX

I would like to create a responsive full form based on the design attached.
The form includes 26 steps including the result page.
The form includes 6 fields related to the user who will fill the form
The first step will include 2 form options to select from the one we will create now and one will do after doing this one.
The form includes 20 multiple choice questions ( 5 choices ) with different points for each choice from (1, 3, 5, 7, 10) each question belongs to a category called Attribute and each attribute belongs to a category called Factor.
The form will include 2 result pages at the end of the form.
The transition between the question should be made without reloading, the design is missing the next and the previous buttons so we should add them.
The form should look like a program and be responsive to all sizes and be easy to be modified
The submissions of the form should be available in the admin and can be exported by excel.
You can use any form builder you want or build it custom from scratch (Provide the form builder name and i will get it or any plugin that will reduce the amount of work and give better results)
The form should be easy to adjust as regular updates and changes might happen.
I have built the form before and this is an update on it the update is mostly on the design part and the jquery and ajax you can continue or start from scratch the form is build using GRAVITY FORMS with custom code for the point calculation and the output.

The first step includes the personal information for the person who will submit the form (Name, Industry, Category, Company Name, Email Address, Phone Number And 2 button act like radio each will redirect to different form we will start now with one form and then we will do the other one on the next button )
*There is an issue in the design the Email and Phone should be placed as the last 2 fields instead of Category and Company)
The second and third steps will include some informative text only
From the 4th step till 23 steps will include questions about the person who submits the form, one question per page multiple choice of 5 choices each will have different point based on the excel sheet attached (points 1, 3, 5, 7. 10), and kindly note that each question belong to Attribute and each Attribute belong to Factor
The 24 steps will include question Text Field will be saved for us only don't have points and here it will be the last question and next button will submit the form and go to the results pages
The 25 step is result page it will show statement based on the points scored from the questions before, there will be 3 statements based on the collected score ( Under 120 points, between 121 and 139 points, Above 140 points ), everything included in the excel sheet.
The 26 Step will show charts and table represent the performance based on the score for Factor and Attribute
It will include 3 Pie Charts based on the Factor score from the overall score
The table will represent the score based on the Attribute sum score circle will be shown under the header (Poor, Fine, Good)
