Word Press Form Generator Plugin using single API GET request

as the title say i am searching for a telnet word press Plugin Developer who can create a word press plugin for me , this plugin should generate a form Box with accepts inputs from user and then by using the API the plugin should be able return a single values to show it to the user .
admin should be able request this generated forms on any page on the WordPress site .
the plugin should support both desktop view & mobile view with no design bugs or problems .
