Web Form Questionnaire (Interactive Format)

I’m looking for someone that can build a well functioning questionnaire that I can offer on my website, the questionnaires is a score questionnaire which will automatically generate the result for me to see.

The questionnaire is ready. It is in pdf form as well as the scoring sheet.
They need to be transferred into an interactive format.
