Urgent Troubleshooting of Email SignUp Page

Hello I added a email sign up to two of my web pages on my woocommerce website. When visitors sign up they get 5 free design templates and recently i updated the message in mail chimp and today a customer told me it wasnt work on my website when he tried to add in his email. Not sure if its from the fact that i made a change in mail chimp or if its been like that for a while and i just now realized it.

Im sure i can figure out how to fix this myself considering i already build webistes but i just dont have the time. Looking for someone quick and cheap who can fix this.
