URGENT: Add some form fields and integrate payment options -- 2

Project tasks:
wordpress website using gravity forms. we have a form that needs additional drop down fields about four. and also need to add payment options for following customers.
Able to choose multiple days at a time
Each 3 hour time frame is $35 mornings 9:00am-12:00pm, afternoons 1:00pm-4:00pm
A full day is $65 9:00am - 4:00pm
Once the order is complete we need an order email to us sent to our email, and they need a bounce back response to them on what order summary.
