Troubleshoot WooCommerce Shipping Calculation Issue

We use WooCommerce and WooCommerce shipping for our e-commerce orders.
We also have a plugin that calculates shipping for our Blank Roll Labels separately from the WooCommerce shipping module (see example screenshot of order invoice). This plugin was calculating it's shipping rates correctly until about a month ago but is now only displaying it's default shipping rate of 14.95. It seems something is overriding the calculation of this plugin so it is no longer displaying the correct shipping rate calculations.
We need someone to troubleshoot this for us to determine why the "Hickman Label Distributor" plugin is not calculating shipping properly.
