Streamline Wordpress custom forms and database access

We have a membership based website based on the Wordpress platform.
While the membership aspects are working, some of the database driven features have stopped working since updating php version on the CPanel CentOS server
Need the features to be updated so they work again
The client looks up criteria and the return text field should auto-populate as they type in the criteria.
If the criteria can't be found - suggestions populate underneath
as well as option to submit
