Selection box on form

Hi there I'm trying to find a solution to a single use form selection by way of a number:
Ie: there is 1-14 (blocks/radiobuttons/checkboxes)
when a client reaches the form they select the unit number ie "2"

once they have selected this option and submitted the form then the option "2" is no longer available/greyed out.

I am using Ninja form to create the form in wordpress.
