Need Help with Memberpress on a WordPress Site

We have two items we need help with:
1) We have a memberpress form that allows you to select from a calendar and pick a date but it stores it as (ex: October 17, 2024) instead of (ex: 10/17/2024).
2) We need a checkbox and attestation paragraph added to this form.
