Mailchimp Forms Tagging & Editing on Website

I'm seeking a skilled developer to edit my Mailchimp forms on my WordPress site. Currently, I have about 100 pages with Mailchimp embedded forms. I need these forms tagged appropriately so I can track where each subscriber came from. The tag would be the page name of the page the form is on. ie "Condos For Sale", tag = "#condosforsale"
