we need a form to install on a WordPress website that would create a form the end user can fill out. Some of the options on the form would be populated from a JSON API, and the submit button should push JSON data back to the source database.
The JSON API to pull from and push to has already been created. We just need the form built to allow inputs to it. Ideally the form will be rendered either with Gravity Forms, a bit of mobile responsive javascript, or worst case an iframe.
