Import 4200 record CSV to Wordpress page template you create

I have a CSV file of 4200 records which need to be imported to Wordpress as pages. I do not have an XML file template. I need someone to build a template and import the records.
1. You will build a template file to use, you will have admin access to Wordpress
2. We will look at design and make necessary changes to the page
3. Once complete, you will import the 4200 records from CSV
4. Records should create pages under main pages.
