Forms submit from my word press site

I have PDF contracts that I need to put on my website, I need customers to fill out the information and hit submit, I need a digital signature or something that catches images or even with their phone they can sign. After signing I need to capture the IP address and send them an email and cc my company. Just like DocuSign but on my website, created catchpa so we don't get spam emails
