Fix connection between web form and google apps script macro

I have a macro app script setup on my Google account that performs a number of task including sending entries from Formidable forms to my Google Spreadsheet.

It recently stopped working (My guess is that this is because of an update from formidable plugins.)

I require an expert to take a look at the code, fix it and get it back running.

Please, I don't need an alternative option to connect formidable forms to Google Spreadsheet and the script does other functions including sorting out the data into newly created sheets.
