Dynamic Form - React

1. We have a front-end code that needs to be dynamic in function of the answers. (11 questions only)
2. The Code have documentation to get the answers in a Google Spreadsheet.
3. Needs to be in Spanish, French and English.
4. Will be in a Wordpress as a final step, so, need to make the embed installation once all is confirmen.
