Customize WP plugin: Quiz plugin Phase 1

Please read attached description carefully! The task description is divided into two phases - phase 1 and phase 2. Bidding for this task is only for Phase 1, it does not include phase 2, so please ONLY QUOTE for Phase 1. Even though this task is only for phase 1, I'm hoping to have the same person do phase 2 in the future, so please review the requirements for phase 2 and advise whether its something you can also do. Please note, some details for phase 2 is still abit murky.
