Create a form with formidable forms that requests user information to allow a pdf download.

I would like to use formidable forms to setup a form that is displayed when a download button is clicked. The form requests information like name - email etc and once filled redirects to a new page where the pdf is available for download. Would also like to sent out an automated email with a link to the download. I have the page setup with the button just need to create the form and set it up to give access to the case study and also send the email. I have formidable forms pro set up.
