CRM database with customized fields

We need a CRM to call and manage customers.
-CRM database will have customized fields (please check attachment)
-Order column(filter) and Search function
-export and import (security copy)
-Database must be installed on a local pc (or web hosting)
-Multiple users can connect at same time (LAN)
-Chrome click to call - to phone contacts directly
- Tel/Mobile num field, must force same number structure and tell if contact is duplicated
- Status field must have a diferenciated color.(to distinguiss it)
