WordPress Vendor Registration Form

I need a vendor-registration form added to my existing WordPress site so new suppliers can register quickly and their details land straight in predefined inboxes. The form must capture all information as per excel attached with attached documents.Once a user hits submit, the data should be emailed in a clear excel layout to the email addresses that I can configure in backend. Should be able to update these email ids from time to time. The form will need conditional checks as per fields.

You can use any free Wordpress plugin. What matters most is that the form loads fast, matches the site’s styling and sends reliable email notifications. I will provide WordPress admin access after awarding project.

Acceptance criteria:
• Form visible on the designated page, styled to match the theme
• Required fields as per excel attached with conditional checks.
• Successful submissions delivered to the specified email accounts without going to spam
• A brief success message shown to the user and the page does not break site caching

Once done, let me know and I will test with my own data before signing off.
