I need a MS Word expert to convert a 4 page questionnaire to form fields

I need to convert the attached 4 page document to a form fields document where I can protect and edit it as needed. I would like to be able to tab from field to field and have the input data displayed in Arial Narrow Bold Underlined. It is not copyrighted, we add that so competitors don't use our forms.
