Word document with Drop down text

Hello,
I would like a word document with a drop down box option in 4 columns. I would like if I could have select a word from the options and when I select a word from the selection I get a preselected paragraph with text. I would then like instructions as to how to edit text when required. I have attached a word document as an example of what where i need the drop down control options.
