I’ve built an automated workflow in Replit that should take form data, inject it into a Word template, and then download the finished file. The stumbling block is the current .docx: it doesn’t contain the content-controls or merge-style placeholders the script expects.

Here’s what I need from you:
• Re-create my existing Word document as a clean .docx template.
• Insert clearly-named placeholders for both Text fields and Date fields wherever information must be pulled in.
• Cover every level of detail—from single-line basics like names or simple dates, through moderate personal info, right up to multi-paragraph descriptions—so the automation never misses a target spot.
• Blend different layouts naturally: some placeholders will sit inline inside sentences, others will appear in a small table for tidy data blocks, and longer sections can sit under numbered headings.

Naming conventions should be consistent (e.g., {{ClientName}}, {{StartDate}}, {{Project_Summary}}) so I can map them directly in my Replit script. Please preserve all existing styling and page breaks; only swap the manual text for controls.

Deliverable: a ready-to-test .docx template that drops straight into my workflow and passes a quick Replit fill-and-download test on my side. If you’re comfortable with Word content controls or mail-merge fields, this will be quick work.
