MS Word and Google Docs Creator

I am looking for a freelancer who is capable of creating a number of forms, worksheets, and templates on MS Word and Google Docs.

We have a number of sheets that need to be done and this is an ongoing job. I am attaching a number of examples of what I am expecting for each page. The work will not be exactly as per the images attached but similar.

Please quote for 10 pages to start with. I would need the same page in both MS Word and Google Docs.

Also, send me a sample of MS Word or Google Docs you have created.

As I do not want to entertain automatic bids kindly write MS Word at the top of your bid.

I am expecting an $ amount for 10 (ten) pages NOT per page.
