Stuffed animal birth/ownership certificate

I make home made crochet stuffed animals. They are all custom made for their buyers. I would like to create a certificate of adoption as a template that can be printed out with the owners name to be filled in and a blank area for them to fill with their stuffed animals name. I would also like a small blank area for the general item description. I would prefer if this was in fill format for pdf or a word document.
