Single page MS word document fixed up

I have a form I had created by a Freelancer some time ago and it is not user friendly. It is MS Word and the problem is every time I type data in, the permanent text moves when I don't want it to. I don't want fill-able PDF because every time I start a new form it does not hold the old data and I have to re-do the form from scratch. I would just like a professional looking form that I can enter the data in quickly and easily. Because the Form has personal data I will only share with the Freelancer I select to do the project.
