Retype a Scanned Document

I have an 88 page scanned document that I need completely retype including creating headings (3 levels), TOC, section breaks between each section, recreating flowcharts (not just taking the image from the scanned document), recreating tables, etc. It should be as if it is the original document.

I am not interested in someone just using OCR and spending 20mins to reformat. This needs to be a complete recreation of the document.
