PDF converted to word document (to be edited) & be able to lock it

I am looking for a freelancer who can convert a PDF document to a Word document while preserving the original formatting. The Word document should be editable, but I also want to restrict editing to ensure that the document cannot be altered without my permission.

We would like the below

Client name: to be able to be typed in
Adviser: to be a selection (one we can change quite easily)
Paraplanner: to be a selection (one we can change quite easily)
Date completed: to be a calendar selection

With the checklist on page two, we are wanting these boxes to be selected

We are wanting to be able to edit or ADD more but only if we use the password. Meaning other staff, cant make changes to this without the password


Ideal Skills and Experience:
- Proficient in PDF to Word conversion
- Expert in Microsoft Word
- Familiar with document protection features
- Attention to detail to ensure that the formatting remains the same as the original PDF

Specific Requirements:
- The formatting of the original PDF must be preserved in the Word document
- I will provide detailed instructions for editing the Word document
- The Word document should be restricted to editing to ensure that it cannot be altered without my permission.
