Microsoft Word document 'Quote Template 2022.docx'- add page to exiting word 'Signoff Template R2.docx' to end of 'Quote Template 2022.docx'

I have 2 documents. Microsoft Word
The 'Signoff Template R2.docx' needs to have clickable YES/NO check boxes.
I need 'Signoff Template R2.docx' added to the end of the 'Quote Template 2022.docx' but retaining the format and making the Yes/No boxes clickable.
I then need a single Word Document (saved as a .docx and .dotx) sent back to me which is a combination of the two documents.
