Microsoft Word Book Template Creation

Hello, I am looking for someone to create a Word template for a book. It should be saveable both in docx and odt format ,if possible. See attached an excerpt of the word file. The layout and formatting should in the end look as in the attached pdf file. So you need to set margins, headers, footers, double column display, page numbering, decorative lines etc.
