FAMU Academic Progress Form

I have a form that I used at a previous job and want to change it for my current job. I am looking for basically the same setup but with name changes and a few other changes. I don't have time to ensure it lines up and add the pictures. It can be a WORD doc as well because I have to be able to pull information onto the form each semester from a query.

I hope to change the black area to a lighter color like yellow. I need to take off the group category and add a decent size comment area. ALso, make more space in between as well if possible.
