Create fillable application through microsoft word

I need to send a form to my clients for them to fill out and send back to me, and I want instruction on how to add additional questions or change the original questions at a later date. I need my clients to file out an application with their name address phone email address on one side and on the other half of the form Event Date, Venue Name, Location,,, all in one box Then in the next box "Description of the event" and on the same line "Guest Count" Menu Description Provided by? and contact info? Disposable or non-disposable? Buffet____ Platted___ Family Style___? Appitzers ___? Third box Should ask For weddings only Bridal party arrival time? Descriptions of any additional help you made need with setting or breaking down decoration and equipment? Forth box include "designated venue provide brief description Event end time Fifth box Entertainment Provided "a line and a breif description Alcohol or non alcohol, hard alcohol, beer wine and soda In the for weddings only section I need to add will there be a rehersal the night before. and the 6 box should ask "any other additional details. I also want to be able to add my logo at a later date. I would like it done with micro soft word and duplicate in a pdf form
