Create fillable Word Template from existing Word Document

I have a new word document that is a treatment plan to be used in a counseling practice that I want to turn into a word template with fields that can be filled expand as needed. May also need to integrate a crisis plan within this template that was created with Excel. Approximately 10 pages max. Will upload generic forms for reference and will provide files with edits that I would like to use for final product once we start working on project. Would like to start on July 5th.
