Create a Microsoft word template

We need someone with expertise in word to help us create a template. We have chosen the design, however we are having difficulty creating a template, the formatting keeps jumping around.

We want the cover page/first page to merge the attached two layouts and then the subsequent pages to have standard document formatting with the colour header and footer with page numbers on all pages
