Create Microsoft Word version of fee sheet

I am looking for a freelancer who can create a Microsoft Word version of my fee sheet. The current fee sheet is in PDF format.
Requirements:
- The Word document should have a similar layout to the current fee sheet, but with improvements.
- No specific features are required in the Word document, just a recreation of the current sheet that will allow us to edit text/prices.
